Acute Injury Package — Initial Evaluation + Follow-Up Visit
New injuries within the last two weeks — such as a sprained ankle, strained shoulder, or twisted knee. Complex or multi-joint cases are better served through a Comprehensive Consultation.

Your flat fee covers a complete clinical evaluation, including point-of-care ultrasound at no additional cost when clinically indicated. It also includes a follow-up visit or care coordination within six weeks, referral management if needed, and up to six weeks of direct messaging access through Spruce.
If additional diagnostics or procedures are clinically indicated, Dr. Sri will discuss all options and costs transparently with you before anything is done — no surprises, ever.
If you transition into a LISM Monthly Membership within 7 days of your initial consultation, 100% of your evaluation fee will be applied directly toward your initial membership dues and enrollment fee.

Telehealth Acute Visit
Telehealth Acute Visit — $75
Virtual assessment for acute concerns — history review, mechanism of injury, and clinical guidance. Please note: Virtual visits have inherent limitations. A physical examination cannot be performed remotely, and clinical recommendations are based on history and reported mechanism of injury alone. $75 credited in full toward your in-person visit if seen within one week.
Comprehensive Consultation ( 40-minute dedicated visit)
For multi-joint complaints, sports medicine concerns beyond two weeks, integrative medicine evaluations, second opinions, and medical acupuncture consultations. Telehealth available for integrative medicine and follow-up visits. In-person evaluation required for sports medicine.

6-Week Care Package Add-On (Available to add during your visit or within 7 days following your consultation): Extends Spruce text access with Dr. Sri to 6 full weeks for protocol/plan tweaks, exercise progressions, and recovery check-ins. Also Includes one 20-minute Follow-Up Visit (in-person or telehealth) at weeks 4–6 to review progress.

A comprehensive pre-participation evaluation that goes beyond the checkbox. Every screening follows the evidence-based American Heart Association and American College of Cardiology framework — ensuring your athlete is cleared safely, not just quickly. All required school, club, and league paperwork is completed and signed at the time of your visit. What happens if a cardiac concern is identified: If a cardiac red flag is discovered during the exam, an immediate diagnostic EKG may be performed right in the room — interpreted under the International Criteria for Electrocardiographic Interpretation in Athletes, the evidence-based standard designed specifically for athletic populations. Depending on findings, additional testing or specialist referral may be recommended — Dr. Sri will guide you through every next step.
