LISM's Sports Injury Walk-In is a digital-first, fast-track evaluation service for acute sports and athletic injuries — designed to get you a clear diagnosis and a path forward without the wait.
A Few Things to Know Before You Check In
No Insurance Accepted
LISM is a strictly cash-pay, out-of-network practice. We do not bill insurance. This independence is what allows us to guarantee next-morning availability, unhurried one-on-one time with Dr. Sri, and completely transparent pricing — no surprise bills, ever.
Digital Check-In Only
This isn't a traditional walk-in clinic. Every patient secures their spot through an overnight digital check-in queue before arriving. Each morning opens two guaranteed evaluation slots and one standby slot — all reserved through the same process. The queue opens at noon the day before and closes at 6:00 AM the morning of your visit.
Arriving without a confirmed slot means we cannot guarantee you a walk-in appointment that morning — but we'll do our best to accommodate you if an opening is available.
One flat fee. Complete care, start to finish.

Your flat fee covers a complete clinical evaluation with point-of-care ultrasound included at no additional cost — plus a follow-up visit or care coordination within six weeks, referral management if needed, and direct messaging access through Spruce for up to six weeks.
A $25 deposit is required to secure your slot through the digital check-in queue. This is applied toward your visit fee if you arrive — and forfeited if you don't.
A convenience fee applies to all Saturday walk-in appointments — $75 for student-athletes and $100 for adult patients, reflected in the Saturday pricing above.
If additional diagnostics or procedures are clinically indicated, Dr. Sri will discuss all options and costs transparently before anything is done — no surprises, ever.

Imaging & Surgical Access
X-Rays Are Not Performed On-Site
X-rays are not included in the baseline visit fee. If imaging is clinically indicated, we coordinate with trusted local radiology partners who offer straightforward cash-pay pricing — typically $100 to $200 depending on the views required.

As a Voluntary Faculty member at the University of Kentucky, Dr. Sri maintains direct relationships with the region's leading orthopedic and sports medicine surgeons. If your injury requires surgical evaluation, LISM will personally coordinate your referral and connect you directly to the right specialist — without delay.
Your 3 Steps to Fast-Track Care
Step 1 — Check In
Secure one of two guaranteed morning slots — 8:00 AM or 8:30 AM — through our secure, HIPAA-compliant check-in form, followed by onboarding through our EMR. You will be asked a few quick questions about your injury — what happened, where it hurts, and how it occurred — so Dr. Sri can review every case before you even arrive. A $25 deposit is required to secure your slot and will be credited toward your visit. Please note: missed appointments forfeit the deposit.

- Saturday walk-ins
- Check-in opens Friday at 12:00 PM and closes at 6:00 AM.
Step 2 — Arrive 20 Minutes Before Your Slot
Please arrive 20 minutes before your scheduled appointment time. Our team will check you in and ensure Dr. Sri has everything he needs before your evaluation begins. This buffer exists to keep care moving efficiently for every patient.
Step 3 — Get Diagnosed and Leave With a Plan
You leave with answers, a clear diagnosis, and a path forward — not uncertainty.
Step 4 — Didn't Secure a Slot?
If the walk-in queue is full, same-day acute injury consultations may still be available throughout the day — Monday through Friday.
