LISM's Sports Injury Walk-In is a digital-first, fast-track evaluation service for acute sports and athletic injuries — designed to get you a clear diagnosis and a path forward without the wait. LISM operates as a direct, cash-pay practice and does not accept insurance.
A Few Things to Know Before You Check In
No Insurance Accepted
LISM is a strictly cash-pay, out-of-network practice. We do not bill insurance. This independence is what allows us to guarantee next-morning availability, unhurried one-on-one time with Dr. Sri, and completely transparent pricing — no surprise bills, ever.
Digital Check-In Only
This isn't a traditional walk-in clinic. Every patient secures their spot through a digital check-in process before arriving. Each morning offers two guaranteed evaluation slots, reserved through the same check-in form.
Arriving without a confirmed slot means we cannot guarantee you a walk-in appointment that morning — but we'll do our best to accommodate you if an opening is available.
One flat fee. Complete care, start to finish.

Your flat fee covers a complete clinical evaluation, including point-of-care ultrasound at no additional cost when clinically indicated. It also includes a follow-up visit or care coordination within six weeks, referral management if needed, and up to six weeks of direct messaging access through Spruce.
A convenience fee applies to all Saturday walk-in appointments — $75 for student-athletes and $100 for adult patients, reflected in the Saturday pricing above.
If additional diagnostics or procedures are clinically indicated, Dr. Sri will discuss all options and costs transparently before anything is done — no surprises, ever.
Cost of Same-Day Interventions — If Applicable
Every intervention is offered only when clinically indicated — and always discussed and agreed upon with you before anything is done.
Suturing and Toe Nail Removal
Medical supplies are billed at direct cost — typically $20-50 depending on the extent of the repair.

Focused Acupuncture Protocol

Advanced acupuncture protocols and specialized procedures requiring dedicated time are scheduled as a separate appointment.
Orthopaedic Bracing & Immobilization
Orthopaedic Splinting
Splint materials billed at cost — typically $15-40 depending on size and type.

Imaging & Surgical Access
X-Rays Are Not Performed On-Site
X-rays are not included in the baseline visit fee. If imaging is clinically indicated, we coordinate with trusted local radiology partners who offer straightforward cash-pay pricing — typically $100 to $200 depending on the views required.

As a Voluntary Faculty member at the University of Kentucky, Dr. Sri maintains direct relationships with the region's leading orthopedic and sports medicine surgeons. If your injury requires surgical evaluation, LISM will personally coordinate your referral and connect you directly to the right specialist — without delay.

Please note: The weekday check-in form is closed between 12:00 AM and 12:00 PM, Monday through Friday. The weekend check-in form is open Friday at 10:00 AM through Sunday at 11:59 PM.
Step 2 — Secure Your Slot
Complete the form to claim one of our available morning slots. If all slots are full, you'll have the option to join our standby list in case a spot opens up.
Please note: check-in is temporarily closed Saturday from 6:00–8:00 AM while we finalize the morning's appointments.
Step 3 — Show Up Ready
Please arrive 20 minutes before your scheduled appointment time. You'll leave with answers, a clear diagnosis, and a path forward — not uncertainty.
